ACL Brace Types: Pre-Surgery, Post-Surgery, and Long-Term Use

When managing an ACL injury, bracing plays a critical role in stabilizing the knee at different stages of recovery. Choosing the right type of brace depends on timing; before surgery, immediately after surgery, or during return to activity. Here’s a breakdown of the main types:

Pre-Surgery Braces (Functional or Hinged Braces)

Purpose:

  • Provide stability to prevent further injury while waiting for surgery.
  • Help manage instability during daily activities.

Features:

  • Hinged on both sides to restrict dangerous movements (like twisting or hyperextension).
  • Adjustable straps for compression and comfort.
  • Worn over clothing or under loose pants.

Post-Surgery Braces (Rehabilitation or Immobilizer Braces)

Purpose:

  • Protect the graft and control knee movement during early healing.
  • Assist with safe weight-bearing and motion control, typically during the first 6–8 weeks.

Features:

  • Lockable hinges to set flexion/extension limits (commonly 0°–90° early on).
  • Large, rigid design for maximal support.
  • Often worn 24/7 in the early weeks, including during sleep and walking.

Long-Term Sports Braces (Functional or Performance Braces)

Purpose:

  • Provide support and confidence during return to sport. (especially pivoting or cutting activities.)
  • May reduce re-injury risk for high-risk athletes.

Features:

  • Lightweight, low-profile design with strong side supports.
  • Allows full range of motion while stabilizing the joint under load.
  • Custom-fit options for high-impact athletes.

Key Considerations:

  • Not everyone needs a long-term brace. Evidence is mixed on whether they prevent re-tear, but they can help psychologically.
  • Fit matters. Improperly fitted braces can cause irritation, slippage, or reduced effectiveness.
  • Always follow your surgeon or therapist’s advice. Bracing protocols vary based on your surgery, graft type, and rehab progress.
